Consent:
I understand that volunteers at Nourish Up may be working in warehouse conditions and that activities may include but are not limited to lifting, working around heavy moving equipment, and handling damaged food products. I hereby accept and assume full responsibility for any injury I might suffer while volunteering for Nourish Up. I understand that volunteers are expected to follow safety rules and all other rules related to the warehouse. In the event of injury, parents/guardians authorize Nourish Up staff to seek treatment for minor volunteers (under 18 years of age) and to take other action should a medical emergency arise, and I waive and release my right for damages.
Confidentiality:
I recognize the serious and personal nature of this position. I promise and covenant that I will not disclose the identity of any service recipient. I will only share any information about a recipient to the Nourish Up staff member who supervises my work. In the event of my withdrawal or resignation, I will continue to hold in strictest confidence all information related to the work in which I have been involved with at Nourish Up.
Safety:
Nourish Up will take all precautions to provide and maintain a safe environment for its volunteers. Volunteers are expected to follow safety rules and all other rules related to the warehouse or applicable location. Nourish Up accepts no liability for minor volunteers who leave the property without parental or guardian consent.
Media Release:
I hereby give Nourish Up permission to copyright and/or use, reuse and/or publish and/or republish pictures or image of me for the purpose of illustration, advertising, and promoting Nourish Up through any medium.
